To order a repeat prescription online please follow the link below, if you are not registered for systmone online service please come into the reception with identification where the receptionist can set this up for you.
The doctor may allow medication to be repeated without the patient being seen. To request a repeat item tick the appropriate item on the request slip (attached to your prescription). Please then post the slip in the box at the Reception desk or post to the surgery.
You can collect the prescription in person or we will post it out to you (as long as you provide a stamped addressed envelope)
Chemist Collection Service
The Crossroads, Village & Forum chemists will collect your prescription and prepare it for collection from them if you ask for this service when requesting your repeat medication. Please note - only the patient or his/her authorised representative can collect a prescription. Patients' representatives must show a signed authorisation card which is available from the practice.
Repeat medication is subject to regular review by the doctor who may invite you to the surgery for re-assessment of your medication needs. If the word "expired" appears on your slip this means you are due for a review. It does not mean the medication is to be stopped.
Please allow two full working days for repeat prescriptions and three working day for new or not repeat items to be processed and remember to take weekends and bank holidays into account.
