Hospital Admission Risk Scoring

As a practice we want to make sure everybody is receiving the right level of care. For some patients regular visits to A&E and/or admission to hospital have sadly become a regular occurrence, which have a significant impact upon their life and their families. We believe that it is possible to help patients remain in their homes with the dignity they deserve.


However, to do this we need to know who requires a little bit of extra support. As a result we are working with The Greater East Midlands Commissioning Support Unit to see who is at risk of an unplanned trip to hospital. All patients registered at the Surgery will be given a risk score. Those patients at significant risk will be assisted by a dedicated package of care.


If you would prefer for your information not to be used in this manner please collect a form from the reception team and we will ensure this is recorded in your notes.
