Online Services

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. More information including “how to” leaflets and videos of patients and why they are using GP online services are available at www.nhs.uk/gponlineservices

How to sign-up

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up

Alternatively patients can use their smartphones and download the NHS App this is a relatively new service and will give access to the same services.

Welcome to Crossroads Medical Practice

With patient’s needs at the heart of everything we do, our website has been designed to make it easy for you to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register, you’ll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.

PLEASE NOTE - WE NO LONGER HAVE A FAX LINE

Please be aware that from the 1st April 2019 you will no longer be able to collect your repeat prescription from the surgery. Please contact the surgery to set up for a pharmacy of your choice who will be open for you to collect your prescription on an evening or weekend. If you are housebound please contact the pharmacy to arrange delivery.

If you would like to register with us, you can either collect a registration pack from reception or follow the registration form link below this paragraph. Please complete the forms as fully as possible and give as much information as you can, this will help us to ensure that we can process your request promptly.  Once the forms are complete please return them to the surgery.

Access

Disabled parking and toilet facilities are available. Access to all patient areas can be achieved without stairs or steps. Non-patient visitors can also access our meeting room via the stairlift.

The surgery shares a common car park with shops and other services at the village centre. When attending the surgery please allow extra time to park. There are 9 spaces for disabled parking.

We have a Patient Participation Group (PPG) details of which can be found on the Patient Group Page. (also shown on the right panel on the Orange Button)
